CONSENT TO THE PROCESSING OF PERSONAL DATA (APPLICANT)

In connection with my application for the Fellowship Program for Journalists and War Correspondents from Ukraine, I hereby give my consent to the processing of my personal data by the administrator - the Foundation for Polish-German Cooperation with its seat in Warsaw, 37 Zielna Street, 00-108 Warsaw:

My consent to the processing of personal data is voluntary, however, I understand that failure to do so will prevent the processing of my application and my participation in the Fellowship Program. I have been informed that I may withdraw my consent at any time, which does not affect the lawfulness of processing based on consent before withdrawal.

INFORMATION CLAUSE (CANDIDATE)


  1. The administrator of the personal data is the Foundation for Polish-German Cooperation with its seat in Warsaw, 37 Zielna St., 00-108 Warsaw, tel. 22 338 62 00, fax 22 338 62 01, e-mail: fwpn@fwpn.org.pl (hereinafter: the Foundation).
  2. Personal data will be processed for the purposes related to the Fellowship Program for Journalists and War Correspondents from Ukraine (Fellowship), in particular:
    • in order to proceed with the candidate's application for the Fellowship and its evaluation, including sharing the data with the members of the jury, communicating the results of the evaluation, implementing the Fellowship programme, including the payment and settlement of the Fellowship; the basis is the candidate's consent, Article 6(1)(a) of the GDPR*;
    • for the purpose of providing information about the Fellowship Program, the Foundation's other scholarship programs or fellows; the basis is the candidate's consent, Article 6(1)(a) of the GDPR;
    • for the purpose of obtaining a PESEL number for the Fellow, if the Fellow does not have one; the legal basis is Article 6(1)(a) of the GDPR;
    • in order to be transferred to other administrators processing data on their own behalf that are necessary for the processing and settlement of the Fellowship , including, but not limited to, entities involved in obtaining a PESEL number, postal or courier operators, banks or payment institutions, accommodation providers; the legal basis is the candidate's consent, Article 6(1)(a) of the GDPR;
    • in order for the Foundation to fulfil its obligations under generally applicable legislation, in particular tax, accounting, social security legislation; the legal basis is Article 6(1)(c) of the GDPR;
    • for the purpose of investigating, establishing or defending against claims, based on the Foundation's legitimate interest; the legal basis is Article 6(1)(f) of the GDPR.
  3. Consent for personal processing is voluntary, but failure to do so will prevent your application from being considered and your participation in the Fellowship Program. Consent given can be withdrawn at any time, which does not affect the lawfulness of data processing based on consent before its withdrawal.
  4. Personal data will be stored:
    • for the implementation of the Fellowship Program - for the period until the completion of all programme activities, including approval of reports and financial settlements;
    • for public information about the Fellowship Program - in the case of Fellows - until consent is withdrawn, in the case of other applicants - for [5] years;
    • for the purpose of obtaining a PESEL number for the Fellow, if the Fellow does not have one - until the PESEL number is obtained and provided to the Fellow;
    • for the purpose of fulfilling accounting and tax obligations - for the period in accordance with the applicable laws, as a rule 5 years from the end of the calendar year in which the tax obligation arose;
    • for the purpose of investigating, establishing or defending against claims - until the expiry of the period of limitation of claims;
    • for the purpose of transferring them to other administrators processing data on their own behalf, which are necessary for the implementation and settlement of the Fellowship Program - for the period until the end of the programme and settlement of the Fellowship Program.
  5. Personal data may be transferred to authorised bodies and institutions on the basis of legal provisions.
  6. The Foundation may also transfer personal data to entities processing data on its behalf or participating in the performance of processing activities, i.e. entities operating the Foundation's IT systems and providing it with other IT tools; entities providing the Foundation with advisory, consulting, auditing, tax, accounting services. The Foundation may also transfer data to other controllers processing data on their own behalf, i.e. entities carrying out postal and courier activities, banks or payment institutions.
  7. The Foundation does not transfer personal data to third countries (outside the European Economic Area) or to international organisations.
  8. The Foundation will not make automated decisions based on personal data, including decisions resulting from profiling.
  9. Any applicant and Fellow may request from the Foundation access to his/her personal data, rectification, erasure or restriction of processing and may object to the processing of the data, as well as request the transfer of the data to another data controller to the extent compatible with Article 20 GDPR.
  10. A person who believes that the processing of personal data by the Foundation violates the provisions on the protection of personal data has the right to lodge a complaint to the President of the Office for Personal Data Protection, 2 Stawki Street, 00-193 Warsaw, tel. +48 22 531 03 00, e-mail: kancelaria@uodo.gov.pl, electronic mailbox: /UODO/SkrytkaESP.
